Pit and Fissure Caries

During the past three decades, the prevalence of dental caries in schoolchildren has fallen in Western Europe and in the USA. Despite this overall reduction in caries prevalence, there has been a reported increase in the proportion of pit and fissure lesions (see Figure 5.1). This is thought to reflect the protection afforded to smooth tooth surfaces by fluoride. Even in areas with a low concentration of fluoride in the water supply, the level of caries has fallen due to the increased use of fluoridated toothpastes. Occlusal caries have been shown to account for 83% of the total caries experience of children between the ages of 5 and 17 years, with the first permanent molar being the most frequent site of fissure caries. An appreciable number of pit and fissure caries have been found in the late teens and early twenties. This shows that fissure caries continue to develop throughout the teens and early twenties and are no longer restricted to the first few years following eruption. New lesions can develop at any age.
