Aim
Here are some ideas on how to create a questionnaire in order to build up a profile of a young person. Decide on what it is you need to know and choose your questions carefully. Remember, it is worth thinking about what a question will tell you. What you want to know about a young person will be specific to their school, their school’s location and age group. Answers to questionnaires are useful in planning future sessions and will allow you to start putting together a picture of the young person’s life. However, it is important to remember that these questionnaires do not constitute detective work. Do not assume anything about the young person’s home life, which may lead you to make judgements.

How to

Filling in the ‘About home and school’ questionnaire is a good activity to do on your first meeting with a young person. After introductions, ask them to fill out one of the questionnaires included on the following pages or create your own using the type of questions we have detailed below and lots of colour and pictures to make it appealing to the young person. Then go back through their answers and have a discussion around each one.
